Oscar Faguada, PA-C, earned his Master’s in Physician Assistant Studies from the University of La Verne and has been practicing for five years. Fluent in Spanish, he enjoys vascular medicine because it is rewarding to help patients find relief from challenging conditions. At ProvascularMD, he values the supportive team and the opportunity to care for an inspiring patient community. Outside of work, Oscar enjoys reading, art, music, and time with family.

Diana Martinez, MSN, NP-C, earned her Master of Science in Nursing from Azusa Pacific University and has been practicing at ProvascularMD for three years. Fluent in Spanish, she was inspired to pursue vascular medicine after meeting Dr. Lalezarian and seeing his dedication to serving the High Desert community. Diana finds fulfillment in helping patients heal and thrive, and appreciates the mentorship and teamwork at ProvascularMD. Outside the clinic, she enjoys working out, family time with her three dogs, the outdoors, and small faith groups.

Ashley Dorn, MSN, FNP-C, earned her Master of Science in Nursing from Azusa Pacific University and has been practicing for over a year. She is passionate about limb preservation, managing complex vascular conditions, and helping patients maintain mobility and quality of life. Inspired by caring for her grandmother during a serious illness, Ashley brings compassion and dedication to every patient. At ProvascularMD, she values the collaborative team approach to care. Outside of work, she enjoys traveling and spending time with her husband, three children, and their dog.
